Scheduled or Self-Determined: Evaluating the Optimal Implementation of the OR-Stretch, an Intraoperative Microbreak for Surgeons

DescriptionTo address work-related musculoskeletal pain for surgeons, the Human Factors Engineering Lab at Mayo Clinic created the OR-Stretch, a 70-second microbreak surgeons can perform during an operation. 15 surgeons at a single institution completed two operative cases using the OR Stretch, once using a Scheduled (every 45 minutes) timing strategy and once using a Self-Determined (between 30-60 minutes based on surgeon preference) strategy. 8 surgeons preferred the Scheduled timing strategy, and 7 preferred the Self-Determined strategy. The stretch was skipped 16% of the time using the Scheduled strategy and only 6% with the Self-Determined strategy. Remembering or forgetting the stretch was a primary factor in determining continued usage. Neither a Scheduled nor a Self-Determined timing interval is superior for microbreak implementation, thus the intervals should reflect surgeon preference. Efforts to promote long-term adoption of microbreaks should focus on reminding surgeons to use them.
